To schedule your appointment please fill out the contact form and we will set your appointment as soon as possible. You will see the doctor typically within three days.
Please note that Psychological Evaluations for Pain Management must be paid in advance prior to the appointment being set. The fee for this evaluation is $400. This will include a clinical interview, psychological testing and review of medical records. A written report will be sent to your physician within three days. Your insurance may or may not cover the this evaluation not all do. We are happy to provide you with any documentation including reports and copies of treatment invoice for submission to your insurance company. Should we not be able to fulfill this examination and provide you with a written report you will be refunded the total amount paid.
